Essential Duties & Responsibilities- Psychiatric Care and Rounding:
Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings. - Collaborative Care Coordination:
Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care. - Patient Evaluation and Management:
Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories. - Diagnostics and Therapeutics:
Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health. - Medication and Treatment Management:
Prescribe and administer medications and treatments; provide injections as necessary. - Patient and Family
Education:
Educate patients and families on medications, care plans, and preventive mental health practices. - Acute Care
Collaboration:
Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.

- Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active DEA license.
- BLS and ACLS certification.
- Current Nurse Practitioner license in the state of practice.
