Responsibilities Provide direct nursing care, medications, and treatments in accordance with physician orders and care plans. Monitor resident condition and promptly communicate changes to the healthcare team. Accurately document resident care and interventions. Collaborate with physicians, nurses, therapists, and interdisciplinary care teams. Follow infection prevention, safety, and regulatory standards. Deliver compassionate, resident-centered care that promotes health, dignity, and quality of life.

Qualifications Active, current, unrestricted Licensed Practical Nurse (LPN) license in the state of practice. Graduate of an accredited Practical Nursing program. New graduates are encouraged to apply. Experience in skilled nursing, long-term care, rehabilitation, or post-acute care is a plus but not required. Strong communication, critical thinking, and teamwork skills. Ability to provide compassionate, patient-centered care in a fast-paced healthcare environment.
