Provides nursing care as needed or assigned on an on-call basis in the patient's home, primarily during evenings, nights, and holidays.

Full-time, exempt position with a 7-days-on/7-days-off schedule. The workweek runs Friday through Thursday. During the scheduled workweek, hours are 5:00 PM-8:00 AM Monday through Friday and 8:00 PM-8:00 AM Saturday and Sunday

Director of Patient Care Services/Clinical Team Leader

• Graduate of approved school of nursing, preferably with a Bachelor of Science in Nursing.
• Current licensure to practice professional nursing in the State of Texas.
• Reliable means of transportation; valid Texas driver's license; automobile insurance.
• One or more years’ experience in clinical nursing. Emphasis on assessment skills and interdisciplinary approach preferred. One year’s experience as a case manager for Hope Hospice preferred.
• Understanding of family dynamics, conflict resolution, and principles of adult learning.
• Pain and symptom management skills, crisis intervention expertise, and family/patient teaching and counseling skills are required.
• Suitable personal appearance, behavior, attitude, and integrity.
• Works well independently. Deals effectively with people.
