Job Overview

Fairview Health Services is seeking a Nurse Practitioner to join our established primary care practice at M Health Fairview Clinics – New Brighton in New Brighton, Minnesota. This is an excellent opportunity to join a collaborative, multi-specialty clinic.

  • Established primary care clinic with 11 providers, including 7 physicians and 4 advanced practice providers
  • Average patient volume of approximately 16–24 patients per day
  • Opening: 1.0

    FTE (34 patient contact hours) - flexible schedule
  • Fully integrated electronic medical record (Epic)

Responsibilities

The Certified Nurse Practitioner is a member of the primary health care team who works in collaboration with physicians and other medical staff to provide comprehensive health care to patients. The Nurse Practitioner engages in independent decision making about health care needs and provides health care services to individuals. The Nurse Practitioner obtains patient histories, performs physical and psychological health assessments, provides patient and family education and counseling, ensures continuity of health care services, and manages acute and chronic disease conditions.

  • Assess health status for specific patient populations using advanced practice knowledge and skills.
  • Obtain relevant health and medical history.
  • Perform physical examinations based on age and medical history.
  • Perform/place preventative and diagnostic procedures based on the patient’s age and history.
  • Identify health and medical risk factors.
  • Identify acute and chronic health problems.
  • Order and interpret diagnostic results appropriately.
  • Develop and implement an individualized plan of care.
  • Establish practice agreements with collaborating physician.
  • Collaborate with health care team members in developing realistic care expectations.
  • Determine effectiveness of treatment plan with documentation of patient care outcomes.
  • Revise and modify the care plan as needed to achieve health and medical goals.
  • Refer care to other health care professionals and community agencies as needed.
  • Prescribe pharmacologic (including controlled substances) and non-pharmacologic agents.
  • Communicate effectively with patients and families for understanding of diagnosis, treatment plan, and need for follow-up care.
  • Provide education to patients, families or caregivers as needed.
  • Document according to established guidelines to facilitate ongoing quality care in a timely manner.
  • Respond in a timely manner to phone calls regarding patient needs.
  • Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
  • Participate in access planning for applicable area or population.
  • Support appropriate billing for professional service through timely documentation and maintaining knowledge of billing policies and procedures.
  • Adhere to all relevant policies, procedures, and practice guidelines.
  • Assist with preparation for regulatory surveys (e.g., JCAHO).
  • Manage patient care through appropriate use of available resources.
  • Prescribe and order ancillary services appropriately.
  • Utilize referral networks appropriately.
  • Manage chronic health/disease conditions.
  • Provide leadership for clinical and support staff and serve as a resource to support staff, enhancing quality of care.
  • Participate in training, orientation, and evaluation of clinical and support staff.
  • Participate in clinical investigation relevant to specific patient population.
  • Respond to nursing staff educational needs and promote nursing procedures, charting procedures, and review of disease treatment modalities.
  • Serve as consultant and role model for nursing staff.
  • Maintain competence and engage in continuing education.
  • Be active in professional and other appropriate organizations.
  • Demonstrate behaviors that support performance improvement and adapt care to developmental level and cultural differences of the population served.
