The Access Case Manager ensures a seamless intake process for clients across EOSIS Treatment Centers, providing consistent support from initial contact through admission. This role builds relationships with clients, referral partners, and families while facilitating intake decisions, coordinating services, and supporting access to appropriate levels of care across multiple program locations.
