Home Health Aide - Full‑time - 1st Shift (Mon‑Fri)

233 Main St., New Britain, CT 06051 – Monday to Friday, 8:00 AM to 4:00 PM.
Community Mental Health Affiliates, Inc. (CMHA) is a private nonprofit treatment provider with seven locations across New Britain and Waterbury. We partner with clients and the community to promote recovery from mental illness and substance use, treating more than 7,500 adults and children each year. Salary range: starting at $19.00 per hour. Compensation is based on experience, education, skills, internal pay equity, benefits, and paid time‑off package.
Position SummaryProvides non‑medical support to a caseload of older adult clients (55+), enabling them to stay in their homes. Responsibilities include light housekeeping, laundry, shopping, meal preparation, errands, medication reminders, and transportation to medical appointments. Works with Registered Nurses and Clinical Case Managers to observe and document clients’ physical and mental health in the EMR.
Essential Responsibilities- Conduct home visits and support clients with light housekeeping, laundry, and shopping.
- Provide companionship and personal engagement within professional boundaries.
- Assist with meal preparation and errands.
- Remind clients to take medications and keep medication records.
- Assist clients in traveling to and from medical appointments when needed.
- Collaborate closely with the RN and Clinical Case Manager to maintain the lowest restrictive level of care.
- Provide education and support to families about aging, memory, elder services, alternatives, out‑of‑home placements, and related topics.
- Coordinate care with primary and specialty providers according to treatment plans.
- Perform all other duties as assigned.
- High School Diploma.
- Valid driver’s license.
- Certification in MANDT, First Aid, and CPR (to be completed upon hire).
- Participate in annual trainings as assigned.
- 3–5 years of experience working with older adults (55+) in a home setting; knowledge of serious mental health illnesses is preferred.
