Dietitian

Grandview Healthcare Center is seeking a dedicated and compassionate PT Registered Dietitian to join our long-term care team. The Dietitian will be responsible for assessing, planning, and implementing nutritional programs to meet the dietary needs of residents, ensuring compliance with federal, state, and local regulations. This role plays a key part in promoting residents’ overall health, well-being, and quality of life through proper nutrition and education.

Qualifications
  • Current Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR).

  • Connecticut state license or eligibility for licensure required.

  • Prior experience in long-term care or skilled nursing facility strongly preferred.

  • Strong knowledge of clinical nutrition, menu planning, and regulatory compliance.

  • Excellent communication, organizational, and interpersonal skills.

  • Ability to work collaboratively with nursing, dietary, and medical staff.

Responsibilities
  • Assess residents’ nutritional status upon admission and as required by regulation.

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.

  • Monitor residents’ nutritional intake, weight changes, and overall progress.

  • Provide guidance on therapeutic diets and ensure menus meet residents’ medical and cultural needs.

  • Work closely with the Food Service Director and Dietary Department to ensure meals are prepared and served according to residents’ diet orders.

  • Conduct staff training on nutrition, portion control, and safe food handling practices.

  • Participate in Quality Assurance and Performance Improvement (QAPI) activities.

  • Maintain accurate documentation and comply with all applicable Department of Public Health (DPH), CMS, and facility standards.
