Days/Hours of Position

40 hours per week. Days and hours based on position requirements and flexibility required. Position may require working nights/weekends on occasions. Expected schedule:
Monday–Friday 12:00 pm–8:00 pm.

Primary Purpose

The Independent Living Coordinator reports to the Assistant Director of Residential & Individualized Services and functions according to the policies established by the Board of Directors. The Coordinator is responsible for developing and implementing the independent living curriculum, establishing staff support for supportive housing, In Home Supports (IHS), and safety net services, directly aiding in the implementation of enabling technology, and supervising the assigned supportive housing and IHS programs.

The Coordinator remains current with trends and best practices related to providing services for persons with disabilities with the goal of maximizing the potential of each individual.

Essential Functions
  • Develop and implement the independent living curriculum to aid supportive housing candidates in reaching higher levels of independence.
  • Direct and assist staff in planning, coordinating, and implementing comprehensive services necessary for optimal growth, development, and care of individuals.
  • Recruit and supervise direct support professionals working in supported housing, safety net, and IHS services.
  • Provide supervision and support to supportive housing and IHS individuals, including regular team meetings, individual IP meetings, training oversight, and habilitative process oversight.
  • Collaborate with support staff and individuals to increase knowledge of enabling technology and digital literacy, developing expertise in Grandcare and other assistive technology used in CCARC services.
  • Ensure independent living support, training, and education for staff and individuals to provide the safest environment while allowing shared responsibility.
  • Maintain open communication with the Assistant Director of Residential & Individualized Services regarding information, issues, and recommendations related to the supportive housing program.
  • Work collaboratively with the Assistant Director to provide financial oversight and management for all billable hours and expenses in the assigned supportive housing program, including waiver billing and one-time funding.
  • Possess knowledge of existing quality service regulations established by DDS and ensure adherence to quality assurance expectations set by CCARC.
  • Demonstrate effective interpersonal skills that enhance communication between individuals and staff.
  • Provide 24‑hour on‑call administrative availability in case of emergency to supportive housing staff and individuals.
  • Provide regular supervision at supportive housing sites when individuals are at home to ensure consumer satisfaction.
Other Related Responsibilities
  • Work cooperatively and versatile as a team member.
  • Stay informed of and implement current trends and methods in agreement with CCARC policy and procedures.
  • Organize and participate in team meetings within Residential Services as needed, including individual meetings, staff meetings, clinical staff meetings, and interdisciplinary team meetings.
  • Maintain open communication with participants, families/guardians, staff, DDS staff, and all interested parties.
  • Become certified to complete assistive technology assessments. (Other tasks and duties may be assigned as necessary.)
Technology Requirements
  • Computer literate and proficient in Microsoft Office Word, Excel, Outlook, and comfortable using tablets and smartphones and other general office equipment.
  • Ability to work with and understand databases; must learn technical skills.
  • Ability to work with and learn various assistive and enabling technology to increase each individual’s independence.
  • Experience with THERAP, Sandata EVV systems preferred.
  • Bachelor’s Degree in a related field required; minimum 2 years experience in developmental disabilities and supervisory experience preferred. Alternatively, a High School/GED with minimum 4 years experience in developmental disabilities and supervisory experience preferred.
  • Must complete the Agency’s certification and training as required for this position.
  • Must have a valid Driver’s License, be insurable with Agency carrier, and have own vehicle as needed.
  • Ability to follow oral and written instructions.
Physical Requirements
  • Must possess the physical and emotional health needed for effective and efficient completion of duties.
  • May be required to lift, transfer activities, and restrain residents/program participants; may be exposed to risk of injury from abusive/assaultive residents.
