Nurse Practitioner, Advanced Practice Provider
Oak Street Health, part of CVS Health is seeking a Nurse Practitioner to provide effective, equitable value‑based primary care to Medicare adults. The role focuses on patient autonomy, longitudinal care, and population health leadership within an outpatient collaborative team.
Core Responsibilities- Provision of exceptional primary care and conduct office visits for routine and acute issues.
- Administer annual wellness visits, health risk assessments, and other preventative documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, ensuring evidence‑based screening adherence.
- Assist the care team with phone triage, outreach, and home visits as needed.
- Educate patients on conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one specialty:
Family NP, Adult‑Gerontology Primary Care NP, Adult NP, or Gerontological NP. - Active, non‑probationary state Nurse Practitioner license.
- Active DEA license.
- U.S. work authorization.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC, ICD‑10 coding, and health risk assessments.
- Passion for teamwork and cross‑functional collaboration.
- Desire to be part of an innovative population‑health model.
- Bilingual proficiency in applicable areas.
Base annual salary: $95,738.00 – $. The pay range reflects the base hourly rate or base annual full‑time salary for all positions in the job grade.
