Oak Street Health, part of CVS Health is seeking a Nurse Practitioner to provide effective, equitable value‑based primary care to Medicare adults. The role focuses on patient autonomy, longitudinal care, and population health leadership within an outpatient collaborative team.

• Provision of exceptional primary care and conduct office visits for routine and acute issues.
• Administer annual wellness visits, health risk assessments, and other preventative documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership, ensuring evidence‑based screening adherence.
• Assist the care team with phone triage, outreach, and home visits as needed.
• Educate patients on conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one specialty:
  Family NP, Adult‑Gerontology Primary Care NP, Adult NP, or Gerontological NP.
• Active, non‑probationary state Nurse Practitioner license.
• Active DEA license.
• U.S. work authorization.

• Experience in primary care: internal medicine, geriatrics, or family medicine.
• Understanding of HCC, ICD‑10 coding, and health risk assessments.
• Passion for teamwork and cross‑functional collaboration.
• Desire to be part of an innovative population‑health model.
• Bilingual proficiency in applicable areas.

Base annual salary: $95,738.00 – $. The pay range reflects the base hourly rate or base annual full‑time salary for all positions in the job grade.
