Qualifications
  • Graduate of an accredited school of nursing.
  • Current licensure as a registered nurse in the State of Connecticut.
  • 3-5 years recent clinical experience in related setting.
  • Demonstrated competence in the clinical setting.
  • Ability to collaborate with an interdisciplinary team, critical thinking, and leadership skills.
  • BSN preferred.
  • Valid/Current CPR preferred.
Job Summary

The Registered Nurse Shift Clinical Coordinator is responsible for the coordination and oversight of patient care activities for a designated group of patients on their respective shift. The principal domains of the role are patient care, leadership, care management, and education. The Shift Clinical Coordinator is directly responsible for facilitating patient care by coordinating the activities of the nursing team and collaborating with other disciplines.

Essential

Functions

Performs comprehensive patient assessments/reassessments. Facilitates and assures continuity from shift to shift, the plan of care for each patient that is goal-directed, patient outcome based and reflects the physician’s orders. Directs and supervises the patient care activities of the patient care team. Coordinates patient-centered care activities in collaboration with the treatment team to facilitate attainment of patient care outcomes. Demonstrates continual oversight and follow through for a designated cohort of patients.

Nursing Team. Facilitates and mentors staff related to all patient assessments, plan of care, interventions, education of patient, family/significant other, interdisciplinary communication and patient outcomes. Demonstrates expert clinical skills through provision of direct patient care in area of focus, including critical thinking, knowledge of disease processes, complications and interventions, and advanced assessment skills. Collaborates with other frontline leaders, Clinical Managers, Assistant Clinical Manager and other discipline team members to determine effective resource utilization and management of patient care issues.

Provides performance feedback to manager for all team members through assessing competence and observing performance. Coordinates/Participates in the Team Review process in collaboration with the team members. Assures that communication occurs with families regarding patient status and plan of care when clinical changes occur, after team meetings, and/or per family request. Collects, analyzes, and evaluates patient experience in order to anticipate and mitigate potential patient risk and to enhance the professional practice environment at the unit level.

Initiates and participates in quality improvements, applying knowledge gathered from tracking nursing and unit indicators, national benchmark data, and evidence-based research findings to foster improvements in the quality of care and enhanced performance methodology. Fosters effective delegation, teamwork and supports the clinical and professional development of colleagues through mentoring and role modeling. Facilitates orientation of new employees in collaboration with preceptor and Clinical Resource Consultant.

Physical

Demands

This job requires continuous periods of walking, standing, bending, reaching, with periodic sitting. Occasional climbing, squatting, and kneeling as related to patient care. Job requires the ability to push/pull up to 60 pounds to assist with patient transfers. Position occasionally requires lifting patients and carrying items up to and over 75 pounds. In addition, this job requires frequent use of gross, fine motor and sensory skills in order to pass medications, perform assessments and treatments, and handle equipment used in patient care.

Job requires independent problem identification, abstract reasoning and adaptability to changing environments and information. Application of previously acquired knowledge, flexibility of thought and organizational skills are necessary. Quick decision making is required. A high degree of selective attention, discernment, perception and concentration is necessary. Job requires a written proficiency in English, including attention to detail, adherence to all elements of style, and ability to postulate.

A working knowledge and familiarity with professional/medical terms and definitions is required. Job requires a high degree of communication skills including expression of abstract/complex ideas, use of negotiation and active listening, and an ability to dialogue with multiple parties.
