Rapid Rehousing & Homeless Prevention Case Manager (SSVF) – New Britain, CT
Veterans Inc., the largest provider of services to veterans and their families throughout New England, seeks a highly motivated and goal‑oriented professional to fill this role. The case manager is part of the Supportive Services for Veterans and Families (SSVF) Program and will deliver supportive services in homelessness prevention, rapid rehousing, outreach, and employment counseling/placement to veterans enrolled in Veterans Inc.
programs.
- Perform initial intake and assessments to determine veteran eligibility for Veterans Inc. programs.
- Develop individualized service plans for homelessness prevention or rapid rehousing based on veteran input, interests, and needs.
- Assist veterans in researching and accessing services such as VA benefits, employment training/placement, educational support, counseling, housing, medical services, and other public benefits.
- Track and report case‑management progress through case notes and conferences with the Program Manager, coordinating with all funding sources.
- Maintain accurate record‑keeping/documentation and compile data for monthly reporting into the agency database.
- Conduct outreach activities to promote Veterans Inc. programs and engage new referral sources.
- Bachelor’s degree in Human Services, Social Work, or a related field.
- Minimum of two years’ experience in Employment/Training, Vocational Rehabilitation, Counseling, Outreach, or services to the homeless.
- Proficiency in Microsoft Office Suite.
- Ability to pass a CORI and background investigation.
- Compliance with Veterans Inc. COVID‑19 vaccination policy (with religious or medical exemptions considered).
- Valid driver’s license.

Salary: $28.84 per hour. Position is full‑time, entry level, and located in New Britain, CT.
