Base pay range

• Essential duties and responsibilities include:
• Conducts, reviews, and assesses quality of life survey results, develops patient goal setting, completes appropriate follow up to analyze patients  plan to improve scores and quality of life.
• Communicates with interdisciplinary team regarding patient psychosocial issues affecting treatment and wellbeing.
• Counsels new and current patients and families on treatment options and modalities.
• Conceptualizes problems, interventions and the evaluation of interventions with respect to human behavior, personality development, and social environment.
• May be required to cover more than one facility in the area.

• Must have minimum LSW.
• Master s degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

• Licensed or registered in the state of practice as required by state regulations. ACSW/ASW licensure required in the State of California.

This position will be open for a minimum of three days. The Salary Range for the role is $68,000 - $109,000/year. Compensation depends on qualifications, skills, competencies and experience.
