Job Overview

RWJB Health is seeking a highly dedicated Nurse Coordinator for our Heart Transplant Center at our New Brunswick campus.

Responsibilities
  • Coordinate the clinical and administrative process for patients through all phases of transplantation pre‑ and post‑transplant, VAD care pre‑ and post‑implant, and heart failure management
  • Educate patients, caregivers, and staff regarding transplant/VAD/heart failure
  • Collaborate with physicians and multidisciplinary team members to evaluate and manage patients
  • Complete data capture, analysis, and reporting, as well as other administrative functions required for transplant program/VAD program operations
  • Maintain communication with patient, referring physicians, transplant team, and other healthcare providers
  • Provide required information to UNOS, NJ Sharing Network, Intermacs, VAD Connect, and other databases
  • Participate in a rotational schedule for 24‑hour coverage for VAD and heart transplant related updates as directed by cardiologist
  • Maintain communication with UNOS and donor OPOs in a timely manner
Pre‑Transplant Phase
  • Facilitate evaluation of heart transplant candidates, beginning with the transplant evaluation consent
  • Arrange interfacility transfer when necessary, including informing financial coordinator and clinical nurse manager prior to transfer
  • Assess patient and caregiver understanding of heart failure and transplant, and provide continuous education of the disease process, treatments, evaluation process, and requirements
  • Provide clinical expertise in the coordination of care and resources to meet the needs of heart failure or transplant patients
  • Monitor test results and coordinate follow‑up
  • Facilitate rapid completion of transplant evaluation and case presentation at weekly multidisciplinary team meeting
  • Provide required information to UNOS, NJ Sharing Network, Intermacs, VAD Connect, and other databases as required
Heart Failure / VAD Phase
  • Perform nursing assessments and provide education
  • Communicate test results and changes in plan of care per APP or MD orders
  • Provide continued education regarding disease process, treatments, VAD evaluation process, VAD implant surgery, medications, and follow‑up requirements
  • Coordinate monitoring of test results and follow‑up as needed
  • Encourage and facilitate rapid completion of VAD evaluation and case presentation at weekly multidisciplinary team meeting
  • Provide required information to Intermacs, Thoratec Connect, and other databases as required
  • Follow clinical practice guidelines for VAD patients
Post‑Transplant / Post‑VAD Implant Phase
  • Maintain knowledge of inpatient plan of care and coordinate transplant or VAD care
  • Provide post‑transplant or post‑implant education to patient and caregiver
  • Collaborate with multidisciplinary team for discharge planning
  • Identify and respond to physical, psychosocial, and educational needs of patient and caregiver
  • Reinforce education and post‑transplant or post‑implant care compliance
  • Ensure post‑transplant or post‑implant testing, care, and visits are scheduled in a timely manner
  • Follow up on results of testing
  • Support program research activities and maintain current knowledge of clinical and regulatory requirements
  • Participate in QAPI, provide input to clinical protocols, and support program research activities (if not CITI trained)
Qualifications
  • Required:

    BSN; relevant clinical experience in heart transplant, heart failure, or cardiology
  • Experience with VAD patients required for VAD coordinator role
  • Strong communication, organizational, and presentation skills
  • Proficient computer skills
  • Preferred: MSNCCTC certification
  • BLS and ACLS certification
  • Active New Jersey Registered Nurse license
  • Must be able to provide in‑service training to patients, community groups, and staff
  • Must maintain and submit monthly time studies
  • Must maintain communication with donor OPOs and UNOS in a timely manner
