Case Manager – Children's Behavioral Health Program

Earn $20.67/hour

• Coordinate resources and information pertinent to client needs.
• Effectively and efficiently document all coordination of resources.
• Coordinate referrals for client specific discharge planning; ensure family actively participates in creating a multi‑faceted discharge plan representing the family's choice of service providers and community resources.
• Monitor record keeping, compliance, and authorized units remaining, ensuring that client charts are up‑to‑date and include all necessary documentation, including documentation of significant incidents within 24 hours as needed (incident reports).
• Monitor/assess documents safety.
• Complete necessary documentation of each case, complying with the standards set by Merakey, County MHDS/CASSP, OMHSAS, and other licensing / regulatory / payor organizations.

• At least 1 year of experience working with children/adolescents.
• Valid Driver's License and clean driving record.
• Able to work Monday‑Friday with an on‑call rotation.
