What You’ll Do:

• Conduct initial assessments to understand clients’ needs, strengths, and challenges.
• Develop and implement individualized service plans to help clients achieve their goals.
• Provide ongoing support, advocacy, and guidance to clients throughout their journey.
• Connect clients with appropriate community resources, programs, and services.
• Maintain accurate and confidential case records, ensuring compliance with organizational policies.
• Collaborate with colleagues and external partners to deliver comprehensive support.
• Monitor client progress and adjust plans as needed to ensure positive outcomes.

What We’re Looking For:

• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field (or equivalent experience).
• Strong interpersonal and communication skills, with a genuine desire to help others.
• Ability to manage multiple cases and prioritize tasks effectively.
• Proficiency in basic computer applications (e.g., Microsoft Office, case management software).
• Familiarity with community resources and social service programs is a plus.
• A valid driver’s license and reliable transportation.
• Eagerness to learn and grow in a fast‑paced environment.
