Registered Nurse Supervisor - ICF

Purpose:

The RN Supervisor - ICF will align his/her scope of work with Jawonio’s Mission, while incorporating Jawonio’s Core Values as the guiding principle for all work activities.

Summary:

The RN will provide comprehensive nursing services to residents in a 20-person Intermediate care Facility (ICF). Responsible for coordinating care and treatment, completing required reports, providing training to staff in the home, and participating as part of the treatment team.

Essential Duties and Responsibilities include the following:

Conducts ongoing reviews of Individual’s medical records specific to their health needs. Reviews records to maintain an accurate assessment of Individual’s health condition and quality of care received.

Participates in quality improvement activities to ensure that records and documentation meet regulatory and professional standards.

Implements Corrective Action Plans.

Develops and Implements Plan of Nursing Services Develops Nursing Summaries and other Health Related reports as needed

Conduct Formal In-Service and Trainings on health-related topics

Implements & monitors the scheduling of medical appointments as deemed appropriate for Individual’sAssists the LPN/CNA’s as needed, by providing nursing care services and administration of medications when needed.

Oversee medication administration and certification process of all medication certified staff, in accordance with NYS Education Department. Department of Health & OPWDDDMaintains collegial relationships with physician and other health care providers

Submits End of Shift & IDT Reports Additional responsibilities may be assigned based on program needs

Must cooperate and participate fully with Jawonio and/or Justice Center and OPWDD during an investigation or inspection, including making verbal and written statements as requested by the person(s) conducting the investigation.

Perform fit testing on medically cleared employees for N95 respirator masks.

Perform Covid testing on Individuals as needed. Oversee Covid self-testing on Employees as needed

Education/

Experience:

Licensed by NYS to practice as a Registered Nurse & Associates Degree in Applied Science Language Ability:
Ability to read, analyze, and interpret general medical periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of employees, managers, consumers, and families.

Math Ability:
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

Reasoning Ability:
Ability to solve practical problems and deal with a variety of concrete variables in situations. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Software:
Working knowledge of Microsoft Office Suite, Excel, Precision Care, Telemedicine, & EMR.
