** 12/29/ Wooten Blvd SW, Wilson, North Carolina, , United States of America The Patient Education Programs Educator will be instrumental in helping to drive volume of CKD participants educated to the Kidney Smart programby conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving life changing CKD education through Kidney Smart.

Provide telephonic callsupportto CKD participant sin focused areas acrossthecountry including education,scheduling, rescheduling, and targeted Spanish support.

Review, educate and answer questions on all modality options for dialysis including transplant, hemodialysis, PD and HHDProvide local coveragesupportwith opportunities and data entry as needed. . Complete data entry&tasking into data management system.

Other duties as assigned.
