The Registered Nurse is responsible for providing nursing care to patients. Supervise care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Develop, implement, and evaluate an individual plan of care for each client. Perform assessment and re‑assessment of patients. Communicate appropriate information with the interdisciplinary team. Report client concerns and changes in condition to supervisor and/or physician in a timely manner.
Roles and Responsibilities- Provide and coordinate care by assessing physical and behavioral health needs of client, develop and implement nursing care plans, maintain medical records and educate clients and their families about various physical and behavioral health conditions while ensuring client safety at all times.
- Assess client’s status on an ongoing basis and document pertinent information.
- Ensure nursing treatment plans are specific to assessed needs and complete initial treatment plan upon admission.
- Takes physician’s orders (verbal or via telephone) accurately, transcribes and reviews them, ensuring legibility per policy and procedure.
- Accurately administer medications per ordered note, document count of controlled substances, adjust medications under provider’s supervision as needed, and monitor lab results to validate medication compliance.
- Notify physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications.
- Supervise self‑administration of medications using the safety initiative of two means of client identification, and document and report effects of scheduled and PRN medications.
- Maintain accountability of administration, recording, control and security of medications, including narcotics.
- Complete admission and discharge processes efficiently and accurately as directed.
- Consistently confer with treatment team (nurses, therapists, program managers, physicians) regarding client care, attend and participate in unit activities such as therapy groups, family meetings, staffing meetings, and other relevant gatherings.
- Provide education/explanation to clients, families, and co‑workers regarding medication usage and maintenance, common side effects, client’s rights, and other topics necessary for optimal client care.
- Monitor client acuity, census, staffing needs, and make arrangements for coverage as needed, following established guidelines.
- Ensure thorough, accurate documentation is completed for all clients on each shift and daily client assignment sheets are completed.
- Give thorough report to oncoming staff, supervisors, and physicians.
- Provide coverage in emergency services as needed.
- Graduation from an approved/accredited school of nursing.
- One or more years of nursing experience within the clinical area of service provided by the facility (preferred).
- Experience in behavioral health / psychiatric / addiction recovery environment preferred.
- Valid current RN license from the state or from a compact state, where applicable.
- Certified in Psychiatric Nursing preferred.
