OverviewIn 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Our state‑of‑the‑art facility at 55 Lock Street is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15‑bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x‑ray and ultrasound and a full‑service retail pharmacy.
The Clinical Pharmacist works collaboratively with clinicians, pharmacists and support staff to provide seamless, quality care for patients at Yale Health.
Responsibilities- Medication Management
:
Accurately and efficiently prepares and dispenses (and/or directly supervises) medications and pharmaceutical products, ensuring proper labeling, packaging, dosage information, and stock levels. Manage sterile compounding technician and daily IV workflow, in accordance with applicable guidelines and standards. Conduct timely and accurate review of medication orders for appropriateness. Monitor patients' responses to drug therapy and adjust medication regimens as outlined in the CDTM agreement and/or policy.
Counsel patients on the safe and appropriate use of medications. - Clinical Support
:
Provide medication information to internal departments. Support pharmacy operations with development of protocols/guidelines, formulary reviews, and medication use evaluations; and identify opportunities to expand and improve clinical pharmacy services throughout the organization. Support clinical departments to address issues related to medication access and/or functionality of automated dispensing cabinets (ADCs). Provide on‑call coverage as per assigned calendar. - Safety and Integrity
:
Adhere to organizational and regulatory guidelines, standards and best practices. Participate in quality initiatives and organizational committees; including functions that support audits, reporting, evaluation of quality opportunities and concerns, and creation and implementation of corrective actions, as requested. Assist in the precepting and training of other pharmacy staff members, students and residents.
