Overview

In 1971, Yale University established Yale Health to provide health services to its facility, staff and students through a multidisciplinary health maintenance organization located on campus. Our state‑of‑the‑art facility at 55 Lock Street is where our members receive most of their care from our 150+ providers. It is a 144,000 square foot medical facility with over 90 exam rooms, an Acute Care Department, a 15‑bed inpatient facility with two negative pressure rooms, a diagnostic imaging suite including MR and CT scan, x‑ray and ultrasound and a full‑service retail pharmacy.

The Clinical Pharmacist works collaboratively with clinicians, pharmacists and support staff to provide seamless, quality care for patients at Yale Health.

Responsibilities
  • Medication Management
    :
    Accurately and efficiently prepares and dispenses (and/or directly supervises) medications and pharmaceutical products, ensuring proper labeling, packaging, dosage information, and stock levels. Manage sterile compounding technician and daily IV workflow, in accordance with applicable guidelines and standards. Conduct timely and accurate review of medication orders for appropriateness. Monitor patients' responses to drug therapy and adjust medication regimens as outlined in the CDTM agreement and/or policy.

    Counsel patients on the safe and appropriate use of medications.
  • Clinical Support
    :
    Provide medication information to internal departments. Support pharmacy operations with development of protocols/guidelines, formulary reviews, and medication use evaluations; and identify opportunities to expand and improve clinical pharmacy services throughout the organization. Support clinical departments to address issues related to medication access and/or functionality of automated dispensing cabinets (ADCs). Provide on‑call coverage as per assigned calendar.
  • Safety and Integrity
    :
    Adhere to organizational and regulatory guidelines, standards and best practices. Participate in quality initiatives and organizational committees; including functions that support audits, reporting, evaluation of quality opportunities and concerns, and creation and implementation of corrective actions, as requested. Assist in the precepting and training of other pharmacy staff members, students and residents.
Principal Responsibilities
  • Prepares and dispenses medications, performs clinical screening including checking for proper drug prescribed, dosage, drug interaction and instructions to patients.
  • Coordinates and participates in disease management programs.
  • Researches and identifies new medications for use in the inpatient and outpatient areas.
  • Presents information to pharmacy management and medical staff as needed.
  • Updates the formulary and notifies patients as appropriate.
  • Consults with clinicians on dosage, medication interactions and other clinical issues. Counsels patients on correct usage of drugs and on possible drug interactions with other prescriptions.
  • Provides accurate drug information to healthcare personnel.
  • Ensures adherence to policies and procedures, legal and regulatory compliance including Board of Pharmacy, HIPAA, DEA, USP. Supports organizational and departmental efforts in seeking or maintaining accreditation including Joint Commission and other federal and state agencies related to clinical practice.
  • Monitors and maintains records on quantity, quality, and type of drugs utilized.
  • Ensures emergency carts, Acute Care cabinets, and other clinical areas are stocked with proper medication.
