Elara Caring is seeking a Speech-Language Pathologist (SLP) for PRN Home Health in New Haven, CT. You will evaluate patients, determine rehab needs, and develop a plan of care per regulatory guidelines, delivering therapy in the home setting.
Responsibilities include initial and ongoing evaluations, documentation, coordinating with physicians and the care team, and ensuring safe discharge planning. Travel up to 50% is required and patients are in the branch coverage area.
