Overview
The Pediatric Hemophilia RN Coordinator (Per Diem) is a Registered Nurse who reports to Nursing leadership within the Pediatric Treatment Centers and works under the direction of the Attending Physician for the specific specialty or site. The primary role is to ensure seamless coordination of patient care across the continuum, including clinical practice, care coordination, education, and quality improvement. The coordinator acts as liaison between patient, family, provider, clinical staff, pharmacy, and scheduling and registration personnel to support safe and efficient patient flow.
Responsibilities- Lead the health care team to coordinate all aspects of patient care for the specific specialty.
- Provide patient and family education on the medical condition, including signs, symptoms, treatment regimens, side effects, and expected outcomes.
- Collaborate as a key member of the multidisciplinary team across the continuum.
- Participate in data collection related to clinical and operational efficiencies.
- Registered Nurse with a Bachelor's Degree in Nursing preferred;
Connecticut RN license required. - 2‑3 years of nursing experience with knowledge of infusion therapy; ambulatory setting preferred.
- APHON certification preferred, not required.
- Excellent interpersonal and organizational skills; ability to manage multiple priorities and work independently with minimal supervision.
