Licensed Practical Nurse (LPN) | Evening Shift (3p-11p) | Union 1199 Part‑time Position

Advanced Center for Nursing and Rehabilitation is actively hiring a Licensed Practical Nurse (LPN) for the evening shift (3 pm‑11 pm) in a Skilled Nursing Facility located in the New Haven area of CT. Part‑time employees may pick up additional shifts as they become available.

• Order prescribed medications, supplies, and equipment as needed.
• Record health details of residents, including vitals and temperature.
• Carry out direct care to residents based on the care plan in accordance with residents’ needs, nursing procedures, facility policy, and nursing standards of practice.
• Administer medications and treatments to residents as needed.
• Supervise Certified Nursing Assistants.
• Perform assigned duties, including wound care, treatments, and procedures.
• Accurately document observations, interventions, and evaluations pertaining to residents.
• Maintain confidentiality and rights of residents, family members, employees, and facility information in compliance with HIPAA policies.

• Valid Connecticut State LPN license.
• Excellent leadership and communication skills.
