RN Coordinator Intake Malignant Hematology

The RN Coordinator for Intake Malignant Hematology plays a vital role in navigating patients and families through their care journey, ensuring they receive the necessary information and support. This position is ideal for experienced nurses who are passionate about oncology and patient-centered care.

• Location: New Haven, CT
• Facility: Yale New Haven Health
• Schedule: Full-time, Day shift

• Collect Level II patient data for new patients prior to initial visits.
• Develop patient/family education plans based on assessment findings.
• Alert Tumor Board Coordinator for treatment planning sessions.
• Attend treatment planning sessions and document treatment plans.
• Identify candidates for open clinical trials and communicate with the care team.
• Oversee Intake Assistant performance and provide feedback.
• Set and strive to meet annual goals for professional development.
• Perform practice/clinic nurse functions as needed.

Education:

• Bachelor's Degree in Nursing highly preferred;
  Master's Degree in Health-related field preferred.

Experience:

• Minimum of three (3) years experience in adult inpatient services, ambulatory oncology service, or operating room; oncology experience preferred.

Licenses &

Certifications:

• Current Connecticut RN license.
• Oncology Nursing Society certification (OCN, AOCN) desirable.
