Columbus House, Inc. is a non-profit organization based in New Haven, CT, with additional programs in Waterbury and Middletown. Since 1982, Columbus House has been serving individuals aged 18 and older who are experiencing homelessness or are at imminent risk. Moving beyond basic food and shelter, the organization provides life-saving outreach, housing, and comprehensive case management to address root causes of homelessness, including lack of affordable housing, mental health challenges, and unemployment.

Through collaborations with community-based service agencies and the support of volunteers and donors, Columbus House works to help its clients achieve independence and long-term stability.

Role Description

This is a full-time, on-site CCR/FUSE Case Manager position based in New Haven, CT. The Case Manager will provide direct support to individuals experiencing homelessness or at risk, including conducting assessments and providing case management services. Daily tasks include creating individualized service plans, coordinating referrals to community resources, assisting clients in securing and maintaining stable housing, and fostering their journey toward independence.

The role also involves collaboration with community partners to facilitate comprehensive care for clients.

Responsibilities
  • Provide direct support to individuals experiencing homelessness or at risk, including conducting assessments and providing case management services.
  • Create individualized service plans.
  • Coordinate referrals to community resources.
  • Assist clients in securing and maintaining stable housing.
  • Foster clients' journey toward independence.
  • Collaborate with community partners to facilitate comprehensive care for clients.
Qualifications
  • Experience in case management, social work, or counseling; excellent ability to assess client needs and provide individualized support
  • Knowledge of community resources and services related to housing, employment, and mental health
  • Strong organizational skills and the ability to manage multiple tasks and cases effectively
  • Proficiency in documentation, report writing, and maintaining accurate client records
  • Excellent interpersonal skills and the ability to build trust with diverse populations
  • Bachelor's degree in Social Work, Human Services, or a related field (or equivalent experience)
  • Experience working with individuals experiencing homelessness or at-risk populations is a plus
  • Demonstrated commitment to diversity and inclusion; ability to work collaboratively within a team and independently
  • A valid driver’s license and reliable transportation are preferred
