Sage Group Health Care  Staffing is seeking PT for travel/contract position in New Hope, MN. Accepting both local and traveler candidates.

This is a rapid response crisis need offering start and completion bonuses.

Pay Rate:
Range $2400 to $2600 Weekly Gross Pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Assignment Start Bonus: $1250 upon completing 4 weeks of the contract. Contingent on completing the scheduled hours with nocall-outs during the 4-week period

Assignment Completion Bonus : $1250 upon completing the 13weeks of the contract. Contingent on completing the scheduled hours with nocall outs during the 13-week period, or make up for any missed time upon client approval

Rate for the time incurred for driving between the facilities on the same day: $25.00/hr

Unit: SNF Facility

Shift: 8 Hours;
Days, 8.00a - 5.00p

Schedule: 32 hours/week guaranteed and up to 40 hours per week scheduled. 4 x 8-hr or 5 x 8-hr shifts per week, Mon-Fri

Position Requirements:

• 1 year of PT experience
  -Required
• SNF or LTC experience– Highly preferred
• Net Health EMR –Highly preferred
• Covid test 72 hours prior to start date with results issued
  48 hours prior to start date
  - Required
• Must be flexible to float to other facilities within a 50 mile radius on the same dayif needed.

Education, License, and Certification Requirements:

• Bachelor of Science in Physical Therapy or Similar
  - Required
• Current State License as Physical Therapist or Compact if applicable
  - Required
• BLS Certification from AHA or American Red Cross
  - Required (Can be obtained upon offer)

Job Description:

• Provide all aspects of PT Services to assigned case load.
• Implement components of patient care, obtain data related to the treatments provided, andmodify care as necessary.
• Work with PTA’sas required
• Speak with families to find best at-home techniques
• Keep physical therapy area clean and organized
• Complete the documentation using Net Health or similar EMR
• Daily productivity requirement may be up to 90-95%
• Must be flexible to float to other facilities within a 50 mile radius on the same dayif needed.
