IMC is currently hiringa Case Manager
, on a full time basis. The Case Manager is responsible for the coordination of services through the continuum of care that balances the delivery of quality patient care with the cost-effective utilization of resources. Supports collaboration and communication of patient/family and entire healthcare team regarding the patient’s plan of care and continued needs post-discharge and facilitates those services required.
EDUCATION
- Graduate of state approved School of Nursing - Or -
- Graduate of a state approved School of Social Work
LICENSURE/CERTIFICATIONS
- Current license as an RN in the state of Louisiana
- Current license as a LMSW in the state of Louisiana
TRAINING/EXPERIENCE
- Previous experience in Case Management preferred
RESPONSIBILITIES
- Be available by telephone during working hours
- Act as liaison between patients,physicians, and referral sources.
- Be knowledgeable of all assigned patients.
- Conduct on-going clinical reviews toensure appropriate level of care and necessity of continued stay.
- Reports clinical reviews to third party payer sources as required/requested.
- Post authorizations into computer asneeded.
- Make appropriate referrals as ordered per MD.
- Ensures that the most effective andlevel appropriate discharge plan is in place for all assigned patients to promote safety in the post-acute setting.
- Actively involved in interdisciplinary rounding meetings.
- Complies with all compliance expectations.
- Knowledgeable of all CMS Mandated Forms.
