Social Worker

Location:

Hopewell, NJ 08560. Contract Duration:
Two (2) years. Full-time (35 hrs/week) or Part-time (15 hrs/week). 100% onsite.

The Social Worker will provide mental health counseling, case management, discharge planning, and reentry coordination for individuals housed in the correctional facility. This role supports continuity of care for incarcerated individuals by addressing mental health needs, coordinating services, and facilitating successful transitions back into the community.

Responsibilities:

• Provide individual mental health counseling to incarcerated individuals.
• Conduct psychosocial assessments and ongoing evaluations.
• Develop, implement, and monitor individualized treatment and service plans.

• Manage mental health cases from intake through release.
• Coordinate discharge planning and continuity of care.
• Assist individuals with reentry planning, including linkage to housing, healthcare, and social services.

• Collaborate with medical staff, mental health clinicians, and correctional teams.
• Liaise with community-based treatment providers, social service agencies, and reentry programs.
• Ensure smooth transitions between correctional and community-based care systems.

• Maintain accurate and timely clinical documentation.
• Ensure services comply with applicable regulations, policies, and professional standards.
• Participate in interdisciplinary meetings and case conferences as required.

Qualifications:

• Bachelor's Degree or Master's Degree in Social Work (Required)

• New Jersey Social Work License (Required)
• Licensed Clinical Social Worker (LCSW) – Preferred

• Experience providing mental health counseling, case management, discharge and reentry planning.
• Experience in correctional facilities or similar institutional settings strongly preferred.
