The Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution and clinical services ensuring the safe and effective utilization of medications. The Pharmacist is responsible for the dispensing and monitoring of medication therapy in accordance with all applicable regulations and standards.
Responsibilities- Complies with and enforces all state and federal regulations and HMH and site-specific policies and procedures relating to pharmaceutical care and medication management.
- Review and verification of all medications prior to dispensing from the pharmacy department, including allergies, polypharmacy, drug-drug interactions, drug-food interactions, and clinical appropriateness. Communicate with the provider if necessary.
- Oversight of dispensing, chain of custody, and accountability of controlled dangerous substances.
- Appropriate handling of investigational drugs at applicable sites.
- Adheres to USP 795, 797, and 800 standards for sterile and nonsterile compounding and hazardous drugs.
- Oversight of all activities of ancillary personnel (Pharmacy Technicians and Assistant/Couriers).
- Employs safe medication practices as recommended by the Institute for Safe Medication Practices, Joint Commission, and HMH guidelines, policies and procedures.
- Identifies patient population needs and delivers care that is specific to those needs (age, culture, language, hearing/visual impairment, etc.). Communicates with patient, parent, and/or primary caregiver at their level.
- Provides pharmacological information by researching and answering questions of other healthcare professionals.
- Participates in pharmacy clinical initiatives (e.g., renal dose adjustments, IV to PO conversions, pharmacokinetic dosing, antibiotic stewardship, opioid stewardship). Pharmacists are available for patient counseling upon request.
- Responsible for training pharmacy staff and residents; educate pharmacy students and interns on rotation.
- Completes network and departmental competencies, licensing and certification requirements.
- Performs or oversees unit inspections to ensure adherence to medication management standards and proper storage and security of medications.
- Other duties and/or projects as assigned. Adheres to HMH organizational standards of behavior.

Skills and Abilities
Required:
- Graduate of an ACPE-accredited school of pharmacy, or equivalent, with a minimum of a BS in Pharmacy.

Preferred:
- 1-2 years of hospital pharmacy experience.
- Knowledge of USP 795, 797, and 800.
- EPIC experience.
- Registered Pharmacist License.
Minimum rate of $73,028.80 annually. HMH is committed to pay equity and transparency for our team members. The posted rate of pay is a reasonable good faith estimate of the minimum base pay at the time of posting and does not reflect the full value of our market-competitive total rewards package.
In addition to compensation, HMH offers a comprehensive benefits package including health, dental, vision, paid leave, tuition reimbursement, and retirement benefits. Some roles may be eligible for performance-based incentives, bonuses, or shift differentials.
