Overview

The Pharmacist provides comprehensive pharmaceutical care through integrated drug distribution and clinical services, ensuring the safe and effective utilization of medications in accordance with all applicable regulations and standards.

Shift Details
  • 7 days on, 7 days off, 12‑hour shift schedule
Responsibilities
  • Comply with and enforce all state and federal regulations and HMH site‑specific policies related to pharmaceutical care and medication management.
  • Review and verify all medications prior to dispensing, focusing on allergies, polypharmacy, drug interactions, and clinical appropriateness.
  • Oversee dispensing, chain of custody, and accountability of controlled dangerous substances.
  • Handle investigational drugs at applicable sites.
  • Adhere to USP 795, 797, and 800 standards for sterile and nonsterile compounding and hazardous drugs.
  • Supervise ancillary personnel, including Pharmacy Technicians, Assistants, and Couriers.
  • Apply safe medication best practices as recommended by the Institute for Safe Medication Practices, Joint Commission, and HMH guidelines.
  • Assess and respond to the specialized needs of the patient population served.
  • Provide pharmacological information to healthcare professionals.
  • Participate in pharmacy clinical initiatives (renal dose adjustments, IV to PO conversions, pharmacokinetic dosing, antibiotic and opioid stewardship).
  • Provide patient counseling as needed.
  • Train pharmacy staff, residents, students, and interns in all areas of pharmacy practice.
  • Maintain competency, licensing, and certification requirements.
  • Conduct or oversee unit inspections to ensure medication management standards are met.
  • Perform other duties and projects as assigned.
  • Adhere to HMH organizational standards of behavior.
Qualifications
  • Graduate of an ACPE accredited school of pharmacy or equivalent with a minimum of a BS degree in Pharmacy.
Preferred Experience
  • 1–2 years of hospital pharmacy experience.
  • Knowledge of USP 795, 797, and 800 principles.
  • EPIC experience.
Licenses And Certifications
  • Registered Pharmacist License
Compensation

Starting at $ annually.

HMH is committed to pay equity and transparency. The posted rate of pay is a reasonable good faith estimate of the minimum base pay for this role.
