Weare looking for a compassionate Certified Home Health Aide (CHHA) toprovide assistance in daily living (ADLs) and personal care services (PCS) in patients' homes in accordance with an established careplan.

Responsibilities

Provide health care services in patients' residences/and or facilities
Perform domestic and household tasks
Transport and accompany patients to doctor's office or to hospital
Medication reminders
Assist with clients’ personal care activities
Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
Maintain patients care records and document provided services
Assist patients with mobility and physical therapies/exercises
Instruct and counsel patients and families on diet and exercise
Collect routine specimens
Provide companionship and basic emotional or psychological support

Requirements

Valid New Jersey Certified Home Health Aide (CHHA) certification
Valid driver's license
Willingness todrive the client using the client's vehicle when required
Proven working experience as Home Health Aide ( Newly certifiedHHAs are encouraged to apply)
Ability tomonitor vital signs and to collect specimens
Familiarity withbasic nutrition and personal hygiene standards
Compassionate,dependable, professional, and c aring personality
