Overview

Registered Nurse (RN) Home Health Visits - Per Diem role at LOES-Loving Our Elders and Seniors LLC in Lawrenceville, NJ. Our mission is to elevate the quality of life for seniors by delivering compassionate, personalized, and professional in-home care services.

Shifts:

Assessments range from 1–3 hours.

• Initial Assessments:
  Completing comprehensive evaluations for new clients to determine care needs.
• Routine Home Visits:
  Monitoring and updating client care plans during monthly visits.
• PRN (As-Needed) Visits:
  Responding to client needs with timely assessments.
• Post-Hospital Assessments:
  Ensuring smooth transitions from hospital to home care.
• Recertification Assessments:
  Reassessing care needs every six months to ensure optimal support.
• Supervision of Home Health Aides:
  Overseeing aides during visits to ensure compliance with care plans and quality standards.

• A valid NJ State RN License.
• At least 2 years of experience as a Registered Nurse.
• Current BLS and CPR certifications.
• Fully vaccinated against COVID-19.
• Previous home care experience is a plus but not required.
