Job Description:
Led by the community’s Health and Wellness Director (RN), our Wellness Nurse is a Licensed Practical Nurse (LPN) who provides direct nursing care to residents with an emphasis on holistic wellness. The LPN also helps supervise the day‑to‑day nursing activities performed by Certified Nursing Assistants.

Schedule: The Wellness Nurse (LPN) is a part‑time, non‑exempt position scheduled weekdays and alternating weekends, 11pm–7am and/or 7am–3pm.

Salary: $33.00–$35.00 hourly.

Responsibilities and Duties:

  • Monitors the health, safety, and well‑being of all residents.
  • Assists in training and monitoring of medication administration.
  • Supervises care staff in accordance with state regulations and community policies.
  • Maintains clinical quality assurance in accordance with federal, state, and local standards.
  • Promotes the highest degree of service to residents while demonstrating the company’s mission.
  • Conducts thorough resident assessments.

Qualifications:

  • Graduation from an accredited School of Nursing with a current LPN license.
  • At least 2 years’ experience in long‑term care, assisted living, home health, or hospital settings preferred.
  • Experience working with residents with Alzheimer’s or related dementias.
  • Ability to handle multiple tasks.
  • Knowledge of federal and state regulations and nursing practices.
  • Proficiency in computer skills, Microsoft Office (Windows, Outlook, Excel). Ability to learn new applications.
  • Physical stamina to stand, walk, sit, use fine and gross motor skills, reach with arms, balance, stoop, kneel, crouch, talk, hear, and smell.
  • Requires lifting or carrying up to 50 lbs. Must use proper body mechanics.
