Position: Hospice Registered Nurse- Full Time, Days, Home Care, Somerset County NJ
Location: Basking Ridge

Overview

Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a full-time hospice Registered Nurse to work 8 AM–4 PM in the Morris, Essex, Union, and Somerset County area, including weekends and on-call as required by department. You will work with adult and geriatric patients in home care. You will manage a heavy caseload (up to 4 patients per day); time management is essential while giving each patient the time and attention needed.

Compensation

Entry level offers start at $43.00/hour and increase based upon experience. Mileage reimbursement: $0.70. Internal referral bonus: $5,000.

Job Qualifications/Experience

Required

  • Current NJ RN License
  • BSN
  • NJ driver s license in good standing

Preferred

  • Minimum of one-year experience in acute, subacute or home/hospice care setting
  • Experience using an EMR, excellent critical thinking and communication skills
Duties & Responsibilities

Performs patient assessments and manages hospice care plans and documentation.

  • Accurately assesses patient s appropriateness for hospice care, learning needs and progress toward goals
  • Develops the plan of care for the patient and family

Ensures appropriate utilization of services

  • Maintains visit productivity standards
  • Demonstrates knowledge of insurance reimbursement

Completes discharge assessments

  • Performs laptop transfer of data twice daily
  • Completes admission assessment and transfers within 24 hours
  • Completes re-visit documentation and transfers

Uses age-appropriate teaching methods for patients and families

  • Evaluates teaching plan and revises as necessary
  • Documents patient education tools used

Assumes responsibility for Case Management.

  • Develops/implements/evaluates plan of care based on evidence-based knowledge
  • Revises care plan as indicated
  • Prioritizes care needs
  • Demonstrates knowledge of hospice regulations and insurance reimbursement guidelines
  • Shows evidence of interdisciplinary communication
  • Performs HHA supervisions per standards
  • Ensures appropriate utilization of services
  • Follows scheduling process
  • Communicates progress toward goals with IDT
  • Attends and participates in IDT meetings every 2 weeks
  • Evaluates the plan of care and revises as needed

Maintains safe environment for patients and co-workers

  • Follows infection control measures and reports infections
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications
  • Maintains current medication orders

Attends monthly staff meeting

  • Participates in PI meetings and activities/committees as requested
  • Accepts other assignments as requested

Assumes responsibility for case management.

  • Prioritizes schedule to accommodate patient s needs
  • Documents case management activities
  • Demonstrates knowledge of pharmacology and safe medication management
  • Routinely reconciles medications. Maintain current medication orders.

Participates in Performance Improvement Program

  • Reviews health records and gathers data to support PI activities
  • Attends UR meetings as requested
  • Provides orientation/ mentorship to new employees
  • Cross trains to learn new skills and demonstrates a willingness to teach other

Assists with growing agency business

  • Provides orientation to new employees
  • Accepts other assignments as requested
  • Cross trains to learn new skills
