What Your Day Looks Like (LPN Responsibilities) Community Integration and Client Empowerment- Build trusting, positive relationships and align individual needs with their goals.
- Act as a passionate advocate, empowering individuals to advocate for themselves.
- Identify opportunities for greater independence in work‑related tasks.
- Provide direct nursing care, including monitoring vital signs, mobility assistance, and personal care.
- Administer medications accurately in accordance with NJ state regulations.
- Perform basic nursing procedures such as injections and tube feedings (if applicable).
- Participate in the development and implementation of Individualized Support Plans (ISPs).
- Advocate for individuals and collaborate with families, care teams, and community resources.
- Complete timely, accurate documentation of care, treatments, and medication administration—an essential skill in every nursing role.
- Implement positive behavior support strategies.
- Utilize de‑escalation techniques to maintain a safe, supportive environment.
- Active New Jersey LPN License (required).
- A passion for supporting individuals with I/DD.
- Strong communication, professionalism, and time‑management skills.
- A valid driver’s license and reliable transportation.
- Ability to pass background checks per NJ regulations.
