RN - Hospice

New Jersey

This is a 13-week contract position.

AM Days primarily, 5 shifts of 7.5 hours

We are seeking a dedicated Registered Nurse to provide hospice care in a home setting. The ideal candidate will possess strong critical thinking and communication skills while demonstrating a profound understanding of patient care in hospice settings.

• Perform accurate patient assessments, ensuring suitability for hospice care.
• Develop and implement plans of care for patients and families.
• Maintain visit productivity standards and ensure appropriate service utilization.
• Document and complete necessary assessments and revisit documentation within specified time frames.
• Utilize age-appropriate teaching methods and document educational tools used.
• Manage case management activities and prioritize patient care needs.
• Participate in interdisciplinary communication and team meetings every two weeks.
• Ensure a safe environment following infection control measures and demonstrating knowledge of medication management.
• Participate in performance improvement programs and support new employee orientation and mentorship.

• Must have 2-3 years of experience as a RN in a home hospice environment.
• Current New Jersey RN License is required.
• BLS certification is required.
• Experience using an EMR system is preferred.
