Registered Nurse Home Care

At SYNERGY Home Care of Teaneck, we're dedicated to providing compassionate and high-quality care to our patients. As a Registered Nurse (RN), you'll play a vital role in delivering exceptional care and service to our clients. We're seeking a compassionate and dedicated RN to join our team.

• Oversee and coordinate the delivery of quality care in accordance with each client's individualized care plan and needs.
• Conduct home visits with clients within designated geographic territories.
• Monitor clients' conditions and communicate any changes or concerns to the appropriate Clinical or Client Services Manager.
• Review, follow up on, and properly document physician orders and changes in the plan of care.
• Perform comprehensive client assessments and reassessments as necessary.
• Coordinate care among clients, caregivers, families, physicians, and other members of the healthcare team.
• Provide ongoing guidance and support to caregivers to ensure care is delivered in accordance with the client's care plan and agency standards.
• Monitor the quality and continuity of services provided to clients.
• Accurately document observations, interventions, assessments, follow-ups, and evaluations pertaining to client care and services provided.
• Identify client care needs and assist in developing appropriate solutions to ensure the highest level of service and client satisfaction.

• Graduation from an accredited and approved nursing program.
• Current and unrestricted Registered Nurse (RN) license in the State of New Jersey.
• Previous experience in home care, community-based healthcare, or a similar setting, with a strong understanding of the unique needs and responsibilities involved in providing care in the client's home.
• Strong clinical assessment, communication, organization, and care coordination skills.
• Ability to work independently, make sound clinical decisions, and effectively coordinate client care.
• Ability to communicate effectively with clients, families, caregivers, physicians, and members of the healthcare team.
