We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our wellness team. The Wellness Nurse will play a vital role in promoting health and well-being among our clients through preventive care, health education, and routine nursing services.

• Provide routine nursing care and wellness assessments to clients.
• Assist in developing and implementing individualized wellness plans.
• Monitor vital signs and document patient health status accurately.
• Administer medications and treatments as prescribed by healthcare providers.
• Educate clients on healthy lifestyle choices, disease prevention, and self-care techniques.
• Collaborate with healthcare professionals to coordinate care and follow-up.
• Maintain accurate and confidential patient records in compliance with regulatory standards.
• Respond promptly to client concerns and provide appropriate interventions.

• Current and valid Licensed Practical Nurse (LPN) license.
• Proven experience as an LPN, preferably in a wellness or preventive care setting.
• Strong knowledge of nursing practices, medication administration, and patient care.
• Excellent communication and interpersonal skills.
• Ability to educate and motivate clients towards healthier lifestyle choices.
• Detail-oriented with strong organizational skills.
• Ability to work independently and as part of a multidisciplinary team.

This position may involve working in clinical settings, community centers, or client homes, depending on organizational needs. Flexibility and adaptability are essential.
