We are currently seeking a Full-Time
Case Manager for our Upper Saddle River program in Upper Saddle River, NJ.
- Asses clients' needs
- Development and implement service plans for clients
- Provide referrals when required
- Responsible for continuity of care and ensuring that appropriate quality services are provided
- Bachelor’s Degree
- 1 year of related experience in social services
- Valid NJ driver's license
- Bilingual preferred
