The role requires coordinating assessments, maintaining records and linking residents to needed services, with a full-time schedule from 9:00 AM to 5:00 PM, Monday through Friday.
Salary ranges from $25.00 to $27.00 per hour, depending on experience, with a comprehensive benefits
