* Summary:

** Monitors, coordinates and evaluates the care and appropriateness of hospitalization; initiates referrals.  Participates in development, implementation and analysis of care delivery standards and outcome measures.

** Requirements:
** Currently licensed as a Registered Nurse in the state of Illinois. BSN preferred. 2-5 years previous Case Management experience preferred.
** Work Shift Details:
** Days, Day shift with flexible days and weekends including some holidays
