* Summary:

** Looking for a Physical Therapist who is wanting to join a motivating, collaborative team that empowers each other and our patients in our Outpatient setting.

We have opportunity to focus on certain specialties or expand your knowledge and share your skills in multiple specialties. We provide all 1 on 1 individualized care to our patients. Access to Shirley Ryan Ability Lab continuing Education! and Medbridge Home Exercise Program! Opportunities for additional profession growth with continuing education and certifications.
** Essential Duties and Responsibilities
** include the following. Other duties may be assigned:
* Evaluates patient and gathers appropriate information to determine accurate clinical diagnoses and treatment plan.
* Establishes appropriate, functional and objective goals based upon clinical examination.
* Devise detailed treatment and care plans for patients.
* Provides exceptional patient care and service.
* The ability to maintain friendly, cordial relations with all clients and employees; maintain a positive work atmosphere by acting and communicating in a manner that results in positive and respectful working relationships.
* Cultivate relationships with patients and the community.
* Provide continuous communication with referring physicians and other referral sources to ensure a consistent level of care.
* Maintain documentation standards that are necessary for compliance.
* Maintains patient confidentiality regarding medical records per company policies and procedures and assures compliance with all HIPAA guidelines.
* Communicates well in a professional setting.
* Perform additional duties as required.
** Requirements
* ** Bachelor’s, Masters or Doctorate Degree from an accredited Physical Therapy Program
* Current State of Illinois Physical Therapy license (or willingness to apply)

The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.

The expected pay for this position is listed below:$38.98 - $49.70
