American Traveler is hiring a Home Health RN with required home health experience and a valid IL RN license for a day-shift assignment.

• Home Health unit providing skilled nursing, infusion management, wound/ostomy care, telemonitoring, and virtual visits
• Uses Matrix EMR with an iPad provided
• Caseload of 24-26 patients within a territory of approximately 15 miles, which may expand if census is low
• Performs a mix of in-home visits and telehealth/virtual visits
• Day shift schedule, Monday-Friday, 8:00 AM-5:00 PM, 40 hours per week
• No weekend requirements
• No on-call requirements
• 2-day orientation, with the first day in-office and the second day in the field

• Active IL RN license required
• Home health experience required
• Strong Med Surg experience with some home health experience may be considered
• BLS certification required or willingness to obtain
• Valid driver's license and car insurance required
• OASIS and start of care experience strongly preferred
• COVID and flu vaccination required, with no declinations approved

• Provide comprehensive initial and ongoing patient assessments covering physical, psychosocial, and environmental factors
• Initiate, reevaluate, and modify the Plan of Care as needed in coordination with physicians and other disciplines
• Supervise CNAs and coordinate care with other healthcare team members
• Complete OASIS documentation and ensure compliance with Federal, State, and Agency regulations
• Assess patient and family educational needs and implement appropriate teaching strategies
• Promote a clean and safe care environment using established error prevention practices
• Mileage reimbursed at the standard IRS rate
• Local candidates preferred; those residing far from the territory may be redirected
• Maximum of 5 days RTO may be requested during the assignment
