Registered Nurse; RN; Home Health

Responsibilities:

• Provide and coordinate patient care using the nursing process and professional nursing standards.
• Conduct initial and ongoing assessments of patients’ physical, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care.
• Coordinate discharge planning with patients, physicians, caregivers, and other healthcare professionals.
• Complete OASIS assessments and required documentation.
• Supervise CNAs and coordinate care with other healthcare workers.
• Educate patients, families, and caregivers on healthcare needs and treatment plans.
• Follow federal, state, and agency regulations.
• Maintain a clean, safe environment and protect patient privacy and confidentiality.

Requirements:

• Home Health experience — Required.
• Active BLS certification or willingness to obtain — Required.
• Valid driver’s license and car insurance — Required.
• Active Illinois RN license; copy must be included with submission — Required.
• Strong Med-Surg experience with some Home Health experience may be considered.
• OASIS and Start of Care experience — Strongly Preferred.
• COVID-19 and flu vaccinations required; declinations will not be accepted.
• Local candidates preferred; candidates must be within reasonable distance of the assigned territory.
