• Conduct health assessments, physical exams, and diagnostic tests.
• Develop treatment plans, focusing on health promotion and disease management.
• Prescribe medications and manage therapies.
• Provide patient education and counseling.
• Manage acute and chronic conditions.
• Perform minor procedures.
• Collaborate with healthcare team members.
• Document encounters in the electronic health record.
• Participate in quality improvement and organizational policies.
• Serve as a resource and participate in educational activities.
• Provide age-appropriate care across the lifespan.
• Manage referrals and follow-up care.
• Engage in ongoing professional development.

• 1-2 years of clinical experience in primary care/internal medicine.
• Active PA or NP certification in family medicine.
• Active, unrestricted CT medical license or in process.
• ACLS/BLS/PALS certification as required.
• DEA License.

40 hours, M-F, 8 am - 5 pm, with a 4-day work week encouraged.

Prefer 1-2 years of clinical experience as a NP/PA in the specialty. APPs must be appointed and credentialed through the medical staff of a YNHHS hospital.
