Position OverviewProvide intake assessments, group therapy and coordinate referrals to appropriate services as recommended. Provide individual and group therapy in a community-based outpatient clinic. Using Care Logic software, the Social Worker will document all clinical services provided in accordance with DMHAS and SCSI expectations. The Social Worker is expected to manage the caseload assigned, including all documentation, in a timely manner, and to attend team meetings and clinical supervisions.
EssentialFunctions
- Provides clinical services to persons enrolled in the outpatient program.
- Describes outpatient services rendered through the completion of progress notes that include description of the nature of the service, the persons response to the therapeutic intervention and its relation to the goals of treatments.
- Provides individual, group and family therapy to an assigned case load.
- Completes treatment plan reviews that include updates on client progress at least every 90 days and discharges summaries whenever a case is closed.
- Documents all client contacts in the client's record.
- Completes all documentation i.e. referrals, treatment plans and reassessments and progress notes in accordance with the agency's QA standards and in a timely manner.
- Provides appropriate linkage with community resources, e.g. social, residential and rehabilitation services.
- Demonstrates comprehensive understanding of all agency programs and program criteria.
- Documentation is legible and relates directly to recovery plan goals.
- Master electronic health record functionality.
- Manages case load, regularly reviews in Care Logic System, performing utilization review independently.
- Attend the mandatory PSRB Acquittee Fundamental Training Program and refresher courses in accordance with DMHAS requirements.
- Understand the responsibilities of community providers when providing services to an acquittee.
- Participates in multi-disciplinary team meetings and Supervision.
- Participates in community awareness presentations as a representative of SCSI.
- Serves on agency committees as needed.
- Responsible for knowing and following all agency policies. This includes knowing and following all the code of conduct policies and the agency's code of ethics.
- Responsible for knowing and following all agency health and safety policies. Responsible for participating in drills and attending all related training sessions.
- Attends 30 hours in-services yearly, maintains State of Connecticut licensure, completes required number of CEUs annually.
- Always maintains confidentiality.
- Meets productivity requirements.
- Will attend adequate clinical supervision by LCSW to achieve successful completion of licensure testing in two (2) years from start date - resulting in LCSW licensure.
- Performs all other duties as assigned.
While performing the duties of this job, the employee is regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs. The employee frequently is required to walk, talk and/or hear;
Speaking and hearing ability sufficient to communicate effectively by phone or in person at normal volumes;
Vision adequate to read correspondence, computer screen, forms, etc. The employee is occasionally required to stand and climb or balance. The employee must occasionally lift and/or move up to 75lbs. Specific vision abilities required by this position include the ability to adjust focus.

QualificationsMaster’s degree in behavioral health science; license eligible; LADC with COD accepted. Experience providing assessments, individual and group therapy, discharge planning, and referral services to adults with severe mental illness, substance abuse and co-occurring disorders.
