Position OverviewThe RN Hospice Nurse ensures quality and safe delivery of hospice nursing services to patients in the home or assisted‑living setting. They develop, implement, and evaluate the plan of care, coordinate interdisciplinary care, supervise licensed practical nurses and home‑health aides, and provide education and counseling to clients and families.
Responsibilities- Develop, implement, and evaluate individualized hospice care plans.
- Coordinate interdisciplinary care and ensure continuity with other health professionals.
- Supervise staff (LPNs, aides) and conduct supervisory visits per regulatory guidelines.
- Provide client and family education on home‑nursing procedures and care needs.
- Maintain accurate electronic documentation (OASIS, orders, time sheets).
- Participate in weekly productivity bonuses and annual salary structure.
- Current RN license required.
- Hospice and/or Home Care experience preferred.
- New graduates will not be considered.
