Recovery Care Coordinator at Patriot Enterprises LLC
The Recovery Care Coordinator (RCC) role supports Air Force wounded warriors, ill and injured service members (RSMs) and their families as they transition back to duty or civilian life.
Job Duties- Advocate for RSMs and families to ensure all needs are met through coordination of services and resources from governmental and non‑governmental agencies.
- Enter and update all RSM case data in the case database and provide monthly status updates on active cases.
- Assist with issues related to health care, benefits, pay, entitlements, legal and medical evaluation boards.
- Collaborate with the Department of Defense, Veterans Affairs and non‑governmental organizations to maximize benefits for wounded warriors and their families.
- Communicate with a multidisciplinary team throughout the continuum of care.
- Aid and guide on all potentially high‑visibility issues that may arise as it relates to RSMs.
- Ensure comprehensive recovery plans are developed and updated in accordance with regulations.
- Collect productivity and performance data.
Skills & Qualifications
- Experience in care coordination/case management.
- Excellent organization and communication skills.
- Interpersonal skills with the ability to work with multiple agencies to accomplish project objectives.
- MS Office proficiency and experience with database systems.
- Frequent travel is required.
- Must be a US citizen.
- Must be able to pass a background check.
- Bachelor’s degree in behavioral or social science, with a preferred emphasis in case management or social work from an accredited US‑based university; and three years of experience in case management or social work.
Skills & Qualifications Desired
- Strong analytical, communication, organizational, and interpersonal skills to work effectively across multiple agencies.
- Previous active duty military experience or experience working with service members or veterans preferred.
