Purpose:
The Registered Nurse is responsible for providing nursing care to patients, supervising care treatment and services provided by LPNs, CNAs, and other staff on the assigned unit and shift. The RN develops, implements, and evaluates an individual plan of care for each patient, conducts assessments and re‑assessments, communicates relevant information with the interdisciplinary team, and reports patient concerns and changes in condition to the supervisor and/or physician in a timely manner.
- Provides and coordinates care by assessing physical and behavioral health needs of patients, developing and implementing nursing care plans, maintaining medical records, and educating patients and their families about various physical and behavioral health conditions while ensuring patient safety.
- Assesses patient status on an ongoing basis and documents pertinent information.
- Ensures nursing treatment plans are specific to assessed needs and are completed upon admission.
- Takes physician orders (verbal or telephone) accurately and documents them; transcribes and reviews orders, ensuring legibility per policy.
- Accurately administers medications per ordered notes, documents controlled‑substance counts, adjusts medications under provider supervision, and monitors lab results.
- Conducts psycho‑education and life‑skills groups as needed.
- Notifies physician of significant changes such as detox difficulties, medical emergencies, or medication response changes.
- Supervises self‑administration of medications using two‑means patient identification; documents and reports medication effects.
- Maintains accountability of medication administration and security, including narcotics.
- Completes admission and discharge processes efficiently and accurately as directed.
- Confers regularly with the treatment team regarding patient care; attends unit activities, therapy groups, family meetings, staffing, and unit meetings.
- Provides education/explanation to patients, families, and coworkers regarding medication usage, side effects, patient rights, and other topics necessary for optimal care.
- Monitors patient acuity, census, staffing needs, and arranges coverage as needed, following established guidelines.
- Ensures thorough, accurate documentation is completed for all patients on each shift and that patient assignment sheets are completed daily.
- Gives thorough report to oncoming staff, supervisors, and physicians.
- Provides coverage in emergency services as needed.
- Graduation from an approved/accredited school of nursing.
- One or more years’ nursing experience within the clinical service area preferred.
- Experience in behavioral health/psychiatric/addiction recovery environment preferred.
- Knowledge of psychotropic medications and medication administration procedures.
- Ability to apply nursing standards and practice within an acute psychiatric setting.
- Valid current RN license from the state or a compact state, where applicable.
- Certified in Psychiatric Nursing preferred.
- CPR and de‑escalation and restraint certification required (training available upon hire).
- First aid may be required based on state or facility requirements.
