Job Description

This position involves delivering clinical pharmacy services from Westfields Hospital & Clinic. The primary objective of this role is to enhance both population-based and patient-specific medication outcomes through collaboration with a multidisciplinary medical team in an ambulatory care setting. The pharmacist is responsible for ensuring safe and effective drug therapy and providing medication management services aimed at preventing or mitigating disease, alleviating patient symptoms, reducing adverse drug events, and improving overall quality of life.
WorkSchedule
- .6 FTE (48 hours per pay period)
- Candidate must have a current Wisconsin pharmacist license (or be eligible) and maintain this license in good standing.
- Candidate must have graduated from an accredited college of pharmacy on or after May 1996 or completed a structured and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire.
- Candidate must have two years of experience providing MTM services as part of a multidisciplinary team within the last five years, or successful completion of a one-year post-doctoral residency focused on providing MTM/ambulatory care pharmacy services.
- Board Certified Ambulatory Care Pharmacist (BCACP) or Board-Certified Pharmacotherapy Specialist (BCPS) preferred.
- This position requires credentialing, following the National Committee for Quality Assurance (NCQA) guidelines.
