Data Collection Coordinator
Our Data Collection Coordinator will be responsible for the issuing of the Important Information on Medicare Notification of Hospital Discharge Appeal Rights (CMS - 4105-F) to Medicare beneficiaries, patients enrolled in Medicare Advantage and other Medicare health plans under MA regulations, dual eligibles, and beneficiaries with Medicare as a secondary payer.
Responsibilities- Collaborate with Case Management, Physicians, Nursing, Patient Access, and Patient Relations to identify all patients who require an IM.
- Issue (within 2 calendar days of the patient’s admission) an IM to the patient or the patient’s representative and obtain a signature if the patient has not received an IM in the Patient Access Department/Admitting. File a copy of the IM in the patient’s chart/record.
- Call the patient’s representative (if not available) on the hospital telephone, and mail or fax the IM on the same day.
- Collaborate with the Nurse Case Manager to identify patients who will be discharged. Re‑issue the IM 48 hours prior to the patient’s discharge.
- Notify the Case Management Department and the Nurse Case Manager of the unit should a patient request an appeal of their discharge.
- Identify and participate in departmental P.I. activities, bringing any discrepancies to the attention of a supervisor.
- A High School diploma or equivalent.
- A minimum of one (1) year of healthcare or customer service experience.
- Excellent communication & interpersonal skills.
- Strong organizational skills.
- Ability to work independently with minimal supervision.
- Bilingual English/Spanish preferred.
USD $25.96 - $26.54 /Hr.
